- The employee ID should be your username (LCOM, UVM or UVMMC whichever you prefer).
- The department administrator provided should be your department chair if requesting a course coordinator/director role, or a MELT-level supervisor if requesting an OMSE role.
- Be sure that only the course(s) which you have directly responsibility for administering are selected.
When you click the "Send Request" button a copy of the request will be sent to the LCOM Registrar's Office and to your supervisor.
